Most recommended documentation can be kept in your household planner, ready to grab if you
need to leave the house in a hurry.
Minimum items include:

Emergency
Notebook/Planner - containing emergency contact info, financial data, etc.

Paper and pencil (a
clipboard with emergency info about your family would be an ideal for rescue
workers if your family leaves your home - see a sample here)

Map of the area- - for
evacuation routes, and to avoid rivers and streams

Notebook
(waterproof if possible) and space pen or 2 pencils

Permanent marker
-
List of phone numbers
-
Prepaid calling card
-
Go-Card for cell phone

Extra set of keys

Homeowners insurance
policy and agent info in a zip-lock bag

Life insurance policies
and agent info

Car insurance policies
and agent info

Copies of deeds, titles
and home inventory

Proof of residence
(driver's license should work) - to get back home

You may reprint and/or distribute these forms as long as you leave the
authoring information intact. I have collected some of these forms for you
